ECD forms and contacts for KwaZulu-Natal

The KZN Department of Education publishes NO ECD registration or subsidy forms on its own website — I enumerated all 151 WordPress pages and all 800 media-library items via its REST API, and the only ECD material that exists is media statements; there is no ECD landing page, and neither the "Forms" nor "Application Forms" pages contain Form 11, 16, 29, 30, GL Form 1, or any subsidy return. What KZN does publish is an official media statement confirming that KZN registration now runs entirely through the national DBE Bana Pele / eCares system (online at user-registration.dbecares.gov.za, a DBE WhatsApp Bot, or USSD published in the statement as "dialling 1347776#"), with support from NGOs and social workers across all 12 districts. The usable paperwork therefore lives on national government domains (education.gov.za, the dbecares.gov.za portals, gov.za), and note that the KZN site's older Joomla-era district-office and district-contact-PDF URLs that still rank in search engines are now all 404 — the department currently publishes no district office contact list at all.

The national route, which works in every province

Registration now runs through the DBE's Bana Pele drive and the eCares system, whichever province you are in. Registration is free — nobody should be charging you for it.

• Register ECD partial care facilities (gov.za service page) — National gov.za service page describing Form 11 - Application for registration / conditional registration of a partial care facility. Lists what the application must contain and the national norms and standards a facility must meet, says it takes up to three months and the service is free. IMPORTANT CAVEAT: this page is out of date - it still routes applicants to the Department of Social Development or a municipal office, which is the pre-2022 process. It explicitly does NOT host the Form 11 PDF ('Form 11 is available at your nearest department of social development or municipal office'). Use it for the requirements list, not the route.
• ECD programme certificate (gov.za service page) — National gov.za service page describing Form 16 - Application for registration / conditional registration of an ECD programme (as opposed to the facility). Lists the required contents of the application, including the implementation plan, staff composition, financial statements, and a Department of Justice clearance confirming no staff name appears in Part B of the National Child Protection Register or the National Register for Sex Offenders. Certificate is granted for up to five years. Same caveat: still describes the old Social Development route and does not host the form.
• Bana Pele 'What documents do I need to apply?' infographic — A 2160x2160 JPEG image (NOT a PDF and NOT a fillable form) from the DBE Bana Pele page. It states the APPLY-stage document requirement: for the owner AND all staff you need (1) certified ID copies and (2) completed Form 30's. This is the only place across the KZN and DBE sites where I actually saw a numbered ECD form named in a live official document. It does not contain the Form 30 itself.
• eCares ECD application portal (user registration) — The live online application form for ECD programme registration - this is what has replaced the paper Form 11 / Form 16 route. Linked directly from the DBE Bana Pele page as 'To apply click here'. It is a JavaScript single-page app (title 'eCares | DBE'), so it must be opened in a browser; there is no PDF to download. Works on phone, tablet or computer with a good connection.
• eCares ECD portal (main entry) — Main eCares application where an already-registered owner signs in to manage their ECD programme profile, upload outstanding COMPLY-stage documents and track the application. Reached by following https://dbecares.dbe.gov.za/ which redirects here. Also a JavaScript app, browser only.
• Bana Pele ECD Registration Drive — National DBE landing page for the ECD registration drive that KZN is running. Explains the three stages (APPLY -> Bronze certificate, COMPLY -> site visit against norms and standards, COMPLETE -> Silver or Gold certificate), states that registration is FREE and warns against paying anyone for registration support, and links the live application portal. This is the authoritative starting point for a KZN creche owner because the province itself publishes no equivalent page.
• DBE ECD programme page — National DBE ECD hub page. Carries the links to the Bana Pele drive and the ECD Info Hub, plus the National Integrated ECD Policy evaluation reports. No registration or subsidy forms are published here - I checked, the DBE's own 'Forms' page (education.gov.za/Resources/Forms.aspx) contains only teacher-award, unemployed-educator and Z83 employment forms, no ECD forms at all.
• KZN Department of Education Head Office contact page — The department's own building-by-building contact page. Confirms Head Office is the Anton Muziwakhe Lembede Building, 247 Burger Street, Pietermaritzburg, and gives named contacts and direct numbers for each building. There is no ECD directorate listed on it - this is the closest thing the department publishes to a directory.
